Who can get services at the School-Based Health Center at Franklin Educational Campus?	Students in grades 7 through 12 currently enrolled at Franklin Educational Campus. For students under 18, parental consent is required. Who provides services at SBHC at Franklin?	Licensed doctors, nurse practitioners, social workers, certified health educator and medical assistants.
When can I get services at SBHC at Franklin? (NOTE: you must have a pass.)	Walk-in Hours: Monday through Friday during the school day, 8:30 a.m. to 2:30 p.m. Schedule an appointment by calling 585-324-3726

What services are available at SBHC?	• Diagnosis, treatment, and follow up for acute and chronic illnesses and injuries. (e.g. sore throats, headaches, sprained ankles, acne, depression, weight issues, anemia, high blood pressure • Complete physical exams for work, sports or school • Immunizations (shots) • Laboratory tests • Counseling for personal concerns • Anxiety screening • Home or school problems • Anger management • Family planning (provide information and guidance) • Routine gynecologic exams • Pregnancy testing and counseling about what to do if you are pregnant; (referrals provided as needed) • Individual health education • Prescription and non-prescription medications
How to pay for services?	NO CHARGE; but if student has insurance, they will be billed
