Who can get services at Threshold Health Center?

Adolescents and young adults 12 to 25 years of age receive confidential care.
Who provides service at Threshold Health Center? Licensed doctors, nurses, nurse-practitioners and certified health educator.
When are services available at Threshold Health Center?  Walk-in Hours:
Monday, Wednesday and Friday 8:30 a.m. to 5:00 p.m.
Tuesday and Thursday
8:30 a.m. to 7:00 p.m.
Saturday 10:00 a.m. to 2:00 p.m.

What services are available at Threshold Health Center?

• Diagnosis, treatment, and follow up for acute and chronic illnesses and injuries. (e.g. sore throats, headaches, sprained ankles, acne, depression, weight issues, anemia, high blood pressure)
• Complete physical exams for work, sports or school
• Laboratory tests
.Counseling for personal concerns
• Family planning -preventing pregnancy and using birth control
• Emergency Contraception
• Routine gynecologic care
• Sexually transmitted disease testing and treatment

• HIV testing
• Pregnancy testing and counseling about what to do if you are pregnant (referrals provided as needed)
• Individual health education
• Prescription and non-prescription medications

 

How to pay for services?

Fees are charged on a sliding scale according to income. Threshold accepts a full range of insurance plans:

  • Aetna
  • Blue Choice
  • Blue Choice Option
  • Medicaid
  • Preferred Care Option
  • Preferred Care
  • Free health care available through the Family Benefit Program, if you qualify. Stop by to check.
• Assistance in enrolling for insurance, many are no or low cost.
